At the completion of treatment please go through this form with the client and record their responses.
This form is for feedback on the service and facilities at the clinic, there is a separate form about the quality of the client's device which you can fill in later.
Exceed P&O Device Feedback Form
Go through this form with the client once they have had time to assess the quality of their device, eg at a later gait training or physio session.
If the client will not be returning to the clinic after receiving device, attempt to contact them via phone to go through this survey.
